Dr Breuningen works in the department of Dermatosurgery (University of Tübingen) Tübingen in which there is a considerable experience in managing patients undergoing these types of procedures. The author can draw from The Tubingen department’s 28 years of experience and large number of publications taken from one of the highest numbers of cases around the world. We are treating over 1500 malignant tumors of the skin per year in the department.

Both surgical skill and histological preparation and evaluation are important for successful treatment of various malignant skin tumors. In addition, good knowledge of local patterns of infiltration is required to achieve complete excision. All tumors may show an irregular, often asymmetric, mainly horizontal subclinical infiltration with a wide extent. When deep infiltrations are present, they are also often asymmetric and hard to determine before excision. Until recently these tumor outgrowths have only been detectable using histologic evaluation, but 3D histology is highly sensitive and can detect even very small tumor infiltrations. Very narrow margins can thus be taken for excision, which helps spare healthy skin as only a subsequently targeted re-excision will be necessary to remove all of the asymmetric tumor infiltrations. Therefore the resulting defect will be as small as possible and as large as necessary, and can be adapted to the requirements for aesthetic surgical defect closure.
