Doctor Yossi Tal is a Social and Cognitive Psychologist, specializing in Risk Management, Safety and Training methodologies. Dr. Tal started his professional career as a Psychologist in the IAF (Israeli Air Force), after graduating in Psychology from the Bar-Ilan University. Dr. Tal headed the branch of Human-Factors in flying for 15 years. He was in charge of investigations and research in various areas of human factors, training, and of the development of recommendations to reduce contribution of human error in military aircraft accidents. For the last 25 years, Dr. Tal has focused professionally on risk management in medicine and patient safety and is a consultant to several medical organizations, including Maccabi Healthcare Services and Leumit Healthcare Services. He has developed, coordinated and instructed numerous academic courses on medical risk management and patient safety and currently he is involved in several research projects in his field of expertise, continues to teach and consult and is active in the board of Israeli Society of Patient Safety and Risk Management in Medicine (NASBAR/ISRM).
Professor Yaron Niv is a Clinical Professor of Medicine at Ariel University in Israel. He graduated from Tel Aviv University Medical School, did his internship and residency in internal medicine and gastroenterology at Sheba Medical Center. He did his postdoctoral work at UCSF and was a visiting professor at Baylor University Medical Center in Dallas, UCSD San Diego, and the Autonomous University of Madrid, Spain. His research concerns the biology and screening of colorectal cancer, Helicobacter pylori-related diseases, mucin secretion in the gastrointestinal tract, quality assurance, and patient safety. He was till recently the Senior Deputy Director General of the Israeli Ministry of Health and responsible for the quality of medical treatment and patient safety. During the COVID-19 pandemic, Prof. Niv served as the Chairman of the National Quality and Inspection Committee for COVID-19 disease patients admitted to general hospitals, Chairman of the National Risk Management Committee for COVID-19 pandemic and immunizations, and Chairman of the National Team for Ventilation and ARDS of Covid-19 Pandemic (2020-2022). He was the founder and president of the Israeli Society of Patient Safety and Risk Management in Medicine (NASBAR/ISRM). He has published more than 300 articles in high-quality peer-reviewed journals.

Medical errors can have serious consequences, often resulting in harm to patients or even death. In the last decades the issue of the 2nd victim was raised, emphasizing the impact of being involved in an adverse event on the ability of caregivers to provide high quality and safe treatment. In 1999, the American Institute of Medicine (IOM) declared that rather than assigning blame for these errors, professional investigations should be carried out to identify what caused them and prevent similar events from reoccurring in the future focusing on systemic factors. It is estimated that in the US alone, there are between 250,000 to 400,000 preventable deaths annually due to medical errors, costing over 15 billion dollars per year.
